The forms below are provided to assist you with providing the school with current information regarding your child at Tuggerah Public School.
Simply download the appropriate form and complete the required fields. Print the form and sign it before returning it to school.
- Absent note form (PDF 4470KB) - If your child is absent from school you are required by law to send a note. We appreciate you contacting the school if your child will be absent for any extended length of time.
- Enquiry to enrol form (PDF 103KB) - Please complete this form if you wish to make an enquiry in enrolling your child at Tuggerah Public School. This is a non-comital form that will provide our office staff with your contact details so we can organise to get information to you regarding enrolment.
- Asthma plan form (PDF 102KB) - Please complete and send this form to school if your child requires a specialised asthma plan.
- Appendix 5 - Declaration for volunteers and non child related contractors (PDF98.3 KB) - Please complete this form if you wish to volunteer in the classroom, sporting events or excursions.
- Application for Extended Leave - Travel and Certificate of Extended Leave - Travel (PDF 226KB). Please complete these forms if your child/children will be away on holidays for 5 days or more.
